An Active-Directive Approach to Behavioral Health

 

I practice Rational Emotive Behavior Therapy (REBT), a form of cognitive behavior therapy developed by the late psychologist Albert Ellis. Unlike some other psychotherapeutic modalities, REBT uses an active-directive approach to behavioral health care. Per one REBT source:

 

REBT therapists believe that it is beneficial for you to provide a structure to therapy and to be active in directing your clients’ attention to salient points that will help them to understand their problems more clearly and that will enable them to do something productive to help themselves.

 

Unlike some modalities which give plenty of space for clients to talk at length about their problems – often uninterrupted and as a means of helping people to feel better – REBT psychotherapists actively direct clients so that people can actually get better.

 

This includes moments of gently interrupting people when they talk, challenging unhelpful assumptions, and using psychoeducation to teach clients how to use effective new beliefs. For context, it may benefit you to know precisely how REBT works.

 

REBT theory uses the ABC model to illustrate how when Activating events (“Actions”) occur and people maintain irrational Beliefs about the events, these unhelpful assumptions – and not the actual occurrences – are what create unpleasant cognitive, emotive, bodily sensation, and behavioral Consequences.

 

In particular, there are four predominate irrational beliefs which people use: demandingness, awfulizing, low frustration tolerance, and global evaluations. Addressing these, the ABC model incorporates Disputation of unhelpful assumptions in order to explore Effective new beliefs.

 

From a psychological standpoint, people disturb themselves using a Belief-Consequence (B-C) connection. Of course, this isn’t to suggest that in the context of the naturalistic or physical world there is no Action-Consequence (A-C) connection.

 

As an example, if a shark bites your leg (Action), you may sustain a serious injury (Consequence). Still, if you unhelpfully Believe, “That shouldn’t have happened to me,” then you’ll probably disturb yourself into an angry or sorrowful disposition (Consequence).

 

Thus, I help people to stop upsetting themselves through use of B-C connections, though I can’t fully resolve their A-C connections. If there were a mathematical formula for the ABC model, it would be something like: Action + Belief = Consequence ÷ Disputation = Effective new belief.

 

Furthermore, this helpful psychotherapeutic modality uses the technique of unconditional acceptance to relieve suffering. This is accomplished through use of unconditional self-acceptance, unconditional other-acceptance, and unconditional life-acceptance.

 

Additionally, foundational components incorporated into REBT relate to Stoicism—a philosophical practice valuing four virtues (wisdom, courage, temperance or moderation, and justice) as a means of achieving eudemonia—a life well-lived, as well as humanism—the process of healing oneself.

 

As well, REBT is influenced by existentialist principles—essentially positing that each of us will inevitably die and that we can search for purpose and meaning as a method of living a well-lived existence. Importantly, all of these techniques require frequent (and I mean daily) practice.

 

Noteworthy, Ellis once practiced psychoanalysis before developing REBT. Of his experience, Ellis stated in an interview, “I concluded in 1953 ‘this psychoanalysis is crap!’ So I gave it up almost completely and went back to active-directive psychotherapy and started to develop REBT.”

 

One key feature of REBT is that homework is negotiated so that clients can practice the techniques they learn in-session outside of a clinical setting. In this way, active-directive practice during sessions can serve as rehearsal for the direct activities in which clients engage outside of sessions.

 

Thus, teaching is a core component of REBT, hence the homework element. In regard to the teaching aspect, one REBT source states:

 

Active-Directive Therapy is significantly more helpful to more people than is passive, less active therapy. Effective therapists not only listen carefully to their clients and not only unconditionally accept them with their problems and with their ineffective, and sometimes antisocial, behavior but also teach them what they did and still are doing to disturb themselves and how they can think, feel, and act differently to ameliorate their emotional and practical difficulties.
